*** NEW FOR 2018 – 2019 ***

Tulpehocken Athletics is going paperless for most of the required PIAA Paperwork.  We hope that this will make things easier for you and will save you some time in the end.  We do realize that since this is a new process this year, there might be some initial confusion.  We hope that the 2018 – 2019 Sports Physical Info Sheet, will help you complete the required 2018 – 2019 PIAA Paperwork with little issue.  Should issues arise, please do not hesitate to contact Liz.  


2018 – 2019 PIAA Paperwork Requirements (Please see the info sheet for more details)

1) Create a Big Teams Account


Click the Online Registration Link (found under or to the right of the photo gallery)


2) Complete the “Sports Registration Requirements 2018 – 2019” on Big Teams

*** We ask that ALL athletes complete this requirement now regardless of what season they will be participating in. ***


3) Complete/Print the Health History (Section 5) and top portion of your Sports Physical (Section 6)

Blank copies of these forms will be available in the Athletic Office if needed.


4) Get your Sports Physical.  Make sure you take your completed/printed Health History and Sports Physical pages to your physical.

Your Sports Physical Form must be completed and dated by the Physician on or After June 1, 2018.  Forms certified before June 1, 2018 will NOT be accepted.  Undated forms are invalid and will be returned.


5) Return your completed Health History (Section 5) and Sports Physical (Section 6) to the Athletic Office BEFORE the season due date.


Fall Sports – Monday, August 6, 2018

Winter Sports – Friday, November 9, 2018

Spring Sports – Friday, February 22, 2018

2018 - 2019 Sports Physicals will take place on Monday, June 4, 2018 from 6:00 PM - 8:00 PM in the Jr/Sr Office/Nursing Office (Please enter through Door #6).  The cost is $15.00.

Athletic Trainers (ATs) are health care professionals who collaborate with physicians to provide preventative services, emergency care, clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions. Students who want to become certified athletic trainers must earn a degree from an accredited athletic training curriculum. Accredited programs include formal instruction in areas such as injury/illness prevention, first aid and emergency care, assessment of injury/illness, human anatomy and physiology, therapeutic modalities, and nutrition. Classroom learning is enhanced through clinical education experiences. More than 70 percent of certified athletic trainers hold at least a master’s degree.

Liz Clark, MED, LAT, ATC grew up in Massachusetts where she was a competitive gymnast and also competed in track and field.  Liz attended West Virginia University where she obtained her Bachelors degree in Athletic Training in 2005.  Liz then went on to attend Alvernia College (now University) where she received her Masters degree in Urban Education in 2007.While at Alvernia, Liz worked as a graduate assistant athletic trainer.  She began working at Tulpehocken in August of 2005.  Although her graduate assistant position was only for 2 years, Liz was fortunate enough to be hired through Commonwealth Orthopaedic Associates and was able to stay at Tulpehocken.  She is now employed through Penn State Health St. Joseph.Liz obtained her Health and Physical Education Teaching Certification From Eastern University in 2008.  She occasionally works as a day to day substitute teacher at the Junior / Senior High School.

Any athlete who sustains an injury or illness that requires them to see a physician needs to return a Doctors note allowing them to return to activity.  You will also need to have your physician complete the PIAA Section 8 Form.  This form should be completed at the time of final clearance and should be retuned to the Athletic Office. [...click for more]
